29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were  made without the manufacturer's written approval: Note:  If such modifications or changes were made, the capacity, operation, and maintenance instruction plates, tags, or decals should have been changed accordingly.    On or about June 24, 2020 at the worksite located at 134 Prince Avenue in Athens, Georgia, employees were exposed to struck by hazards when powered industrial truck operators were observed exposed to injuries resulting from tip-over where the employer allowed for the use of job made trash boxes to be used which could change center of balance.
