29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  On or about 10/16/2020, at the jobsite located at 100 SE 2nd St. Miami, FL 33131, employees were exposed to a chemical hazard when performing janitorial service operations using a corrosive chemical (i.e. Halt with a PH of 11.0-12.0) without wearing eye protection.

29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  On or about 10/16/2020 at the jobsite located at 100 SE 2nd St. Miami, FL, 33131, an emergency eyewash was not available for employees who worked with corrosive chemicals (i.e. Halt with a pH of 11.0 - 12.0 and M95 with a PH less than 1.0 respectively),  exposing employees to a chemical hazard.

29 CFR  1910.1200(h)(2): The employer did not provide employees with effective information on hazardous chemicals in their work area at the time of their initial assignment and whenever a new chemical hazard the employees have not previously been trained about was introduced into their work area. Employees were not informed of the following: the requirements of this section; any operations in their work area where hazardous chemicals are present; and, the location and availability of the written hazard communication program, including the required list(s) of hazardous chemicals, and safety data sheets as required by this section.   On or about 6/16/2020, at the jobsite located at 100 SE 2nd St. Miami, FL, 33131, janitorial employees were not provided with information before accessing a work area being disinfected with Clorox Total 360, which exposed employees to a chemical hazard.
