29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  219 Lakewood Road, South Glastonbury, CT:  On or about June 25, 2020 the employer did not provide guardrail systems, safety net systems, or personal fall arrest systems and did not provide a fall protection plan for its employees. Employees were exposed to fall hazards while walking and working around windows that were between 12 to 18 feet above the ground.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.   219 Lakewood Road, South Glastonbury, CT:  On or about June 25, 2020, employees were exposed to fall hazards, while working close to window openings at heights between 9 feet and 18 feet high and were not provided fall protection training.

1926.1052(c)(3)(ii) Stair rails installed before March 15, 1991, shall be not less than 30 inches (76 cm) nor more than 34 inches (86 cm) from the upper surface of the stair rail system to the surface of the tread, in line with the face of the riser at the forward edge of the tread.  219 Lake Wood Road, South Glastonbury, CT:  On or about June 25, 2020, the handrail between the first and second floor was only 20 inches above the surface of the tread.

29 CFR 1926.404(b)(1)(ii): Ground-fault circuit interrupters. All 120-volt, single-phase 15- and 20-ampere receptacle outlets on construction sites, which are not a part of the permanent wiring of the building or structure and which are in use by employees, shall have approved ground-fault circuit interrupters for personnel protection. Receptacles on a two-wire, single-phase portable or vehicle-mounted generator rated not more than 5kW, where the circuit conductors of the generator are insulated from the generator frame and all other grounded surfaces, need not be protected with ground-fault circuit interrupters.   219 Lake Wood Road, South Glastonbury, CT: On or about June 24, 2020, three saws were powered by an extension cord that was plugged into an ungrounded outlet.
