29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a. On or about 06/29/2020, at 32203 Mahogany Valley Dr. - employees were exposed to an 11 feet 3 inch fall hazard while conducting carpentry work from the second floor of a new house, in that, guard rails were not installed and or fall protection was not used.      b. On or about 06/29/2020, at 32203 Mahogany Valley Dr. - an employee was exposed to a 13 feet 3 inch fall hazard while seating on a window opening ledge to conduct carpentry work, in that, fall protection was not used.      c. On or about 06/29/2020, at 32203 Mahogany Valley Dr. - employees were exposed to a 20 feet 3 inch fall hazard while conducting carpentry work from the roof of a 2-story house, in that, fall protection was not used.

On or about 06/29/2020, at 32203 Mahogany Valley Dr. - employees were exposed to a 11 feet 3 inch fall hazard while conducting carpentry work from the second floor of a new house, in that, guard rails were not installed and or fall protection were not provided with training.     b. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a. On or about 06/29/2020, at 32203 Mahogany Valley Dr. - the rails of the ladder used to access the second floor of a new construction house did not extend at least 3 foot above the upper landing surface, exposing the employees to an 11 feet 3 inch fall hazard.
