29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part: a) Jobsite, Queen of the World Catholic Church, 134 Queen Road in St. Marys, PA. On or about June 26, 2020, the employer did not initiate and maintain safety and health programs, in that the company did not provide its employees with general safety and health rules when working on the construction site. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

Employees working on the roof were exposed to a 20-foot fall. The employee died as a result of his injuries on June 27, 2020. The fatality was not reported to OSHA until June 29, 2020.
