29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:   On or about June 30, 2020, at a worksite located at 519 Erie Street, Painesville, OH 44077, during residential roofing activities on a steep sloped roof, the employer did not ensure employees were protected from eye hazards by ensuring eye protection was available and worn while operating pneumatic nail guns at approximately 120 psi and operating circular saws while cutting plywood.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about June 30, 2020, at a worksite located at 519 Erie Street, Painesville, OH 44077, during residential roofing activities on a steep sloped roof, the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 19.2 feet by utilizing a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about June 30, 2020, at a worksite located at 519 Erie Street, Painesville, OH 44077, during residential roofing activities on a steep sloped roof, employees were exposed to a fall of approximately 15 and 10 feet. The employees were using an extension ladder to access the Equipter trash receptor unit and then another extension ladder to access the roof. Neither ladder was properly set up and did not extend at least three feet above the landing areas.

The employer did not ensure employees were provided safety and health training or that there was a system in place to recognize and correct unsafe working conditions.
