29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation.  San Manuel Casino Project, Section 3 Level 2: On or about January 14, 2020, a worker sustained a fracture to the leg after a concrete hose ruptured during priming operations. Column (F) on the OSHA Form 300 stated: "Knee(s) and leg(s) - Hoses" A description of the injury sustained was not described on the form.
