29 CFR 1926.102(a)(1):   Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  At a commercial worksite located at 12520 I-49 South, Grandview, Missouri. Employees were exposed to serious eye injury while using a Hibachi brand pneumatic nail gun without eye protection.

29 CFR 1926.501(b)(1):   Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At a commercial worksite located at 12520 I-49 South, Grandview, Missouri. Employees were  were exposed to serious injuries, in that the unprotected sides and edges of the elevated work area was not provided with guardrails or any other means of fall protection. The employees were exposed to an approx. 25 to 30 feet fall hazard to the surfaces below.
