Athos Rd. Lynchburg, VA 24504,  the Super Compactor Cell was not classified for flammable hazards where  compacting 55 gallon drums of flammable isopropyl alcohol 99% concentration saturated rags was being performed.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

29 CFR 1910.106(e)(6)(i): "General." Adequate precautions were not taken to prevent the ignition of flammable vapors. Sources of ignition include but are not limited to open flames; lightning; smoking; cutting and welding; hot surfaces; frictional heat; static, electrical, and mechanical sparks; spontaneous ignition, including heat-producing chemical reactions; and radiant heat.   On or about 06-19-2020, located at BWXT NOG-Lynchburg 1570 Mt. Athos Rd. Lynchburg, VA 24504, in the Super Compactor Cell, an employee was exposed to a hazardous flammable atmosphere where 99% isopropyl alcohol was allowed to overflow the trough and accumulate on the floor, and the employer did not take adequate precautions to minimize the ignition of flammable vapors such as cleaning up spills promptly, not compacting metal drums, ensuring electrical outlets and wiring were not damaged, and that electrical equipment was appropriate for the location.       Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.

Maintenance and operating practices were not in accordance with established procedures which will tend to control leakage and prevent the accidental escape of flammable liquids. Spills were not cleaned up promptly.  a) On or about 06-19-2020, located at BWXT NOG-Lynchburg 1570 Mt. Athos Rd. Lynchburg, VA 24504, in the Super Compactor Cell, an employee was exposed to flammable hazards from the compacting of 55 gallon drums containing approximately 25 gallons of 99% concentration isopropyl alcohol where the excess was allowed to pool on the floor for extended periods of time near damaged electrical equipment.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
