Employees were required to use 3M half-face respirator provided by the employer. Employees were not provided with a medical evaluation prior to using a 3M half-face respirator provided by the employer. 29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the jobsite:  (a) On or about July 6, 2020, and at times prior, DJ Maintenance and Construction, LLC., did not ensure that a fire extinguisher was provided within 50 feet of wherever more than 5 gallons of flammable liquids were being used on the jobsite. Employees were exposed to fire hazards while applying D811-23-S decking adhesive, a category 2 flammable liquid, on a deck at a construction jobsite. 29 CFR 1926.152(f)(3): Flammable liquids were used where there were open flames or other sources of ignition within 50 feet of the operation:  (a) On or about July 6, 2020, and at times prior, DJ Maintenance and Construction, LLC., did not ensure that flammable liquids were not used within 50 feet of an ignition source. Employees were exposed to fire hazards while applying D811-23-S decking adhesive, a category 2 flammable liquid, on a deck at a construction jobsite. A Steinel electric heat gun, a source of ignition, was used within 6 feet of the operation.
