(b) On or about July 8, 2020, an employee was repairing a concrete wall from a Genie S-40 aerial lift, was not protected from an approximate 18 foot fall/thrown hazard, by using a safety harness and lanyard attached to the lift.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. (a) On or about July 8, 2020, employees were repairing a concrete wall from a Genie S-45 aerial lift and Genie S-40 aerial lift, had not been trained on the hazards associated with scaffold work and how to mitigate those hazards. This exposed employees to an approximate 18 foot fall/thrown hazard.
