29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  (a)  On or about July 5, 2020, the Single Slice Doboy machine was not adequately guarded to prevent employees from reaching the point of operation, the cutting knife, while threading the cellophane wrapper through the machine.

29 CFR 1910.1030(c)(1)(ii)(B): The employer's written Exposure Control Plan did not include the schedule and method of implementation for paragraphs (d) Methods of Compliance, (e) HIV and HBV Research Laboratories and Production Facilities, (f) Hepatitis B Vaccination and Post-Exposure Evaluation and Follow-up, (g) Communication of Hazards to Employees, and (h) Recordkeeping, of this standard.  (a)  On or about July 5, 2020, the employers written program Body Fluids Cleaning Procedure #50  did not contain procedures for post exposure evaluation and follow up for employees who cleaned blood from machines following employee injuries such as but not limited to amputations.
