29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions:  a) Coaldale, PA: Daily inspections of a 13 foot deep trench were not made by a competent person, which would have identified deficiencies in the cave-in protection system which included improper installation and damage to the protective system which could have resulted in its failure, on or about July 9, 2020.  ABATEMENT CERTIFICATION REQUIRED
