29 CFR  1926.651(b)(3):When excavation operations approach the estimated location of underground installations, the exact location of the installations were not determined by safe and acceptable means:  (a)	On or about June 30, 2020, for the employees exposed to flash fire and electric shock hazards while installing fiber optic lines where the Vermeer D8x12 ruptured the gas line and hit the power line at or near 612 East Burdick Expressway in Minot, North Dakota.
