On or about July 9, 2020, at the facility where employees were exposed to struck-by hazards while operating a Coffing brand two ton hoist that was missing a safety latch on the hoisting hook. c. On or about July 9, 2020 at the facility where employees were exposed to struck-by hazards while operating a Budgit brand two ton hoist that had a defective safety latch on the hoisting hook. d. On or about July 9, 2020 at the facility where employees were exposed to struck-by hazards while operating a Budgit brand one ton hoist that had a missing safety latch on the hoisting hook.
