29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift:  On or about July 10, 2020 and at times thereto, employees working inside an elevated aerial lift were not wearing fall protection and attached to the boom or basket, exposing the employees to being ejected and unprotected fall hazards of approximately 30-feet to the ground below.
