29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the jobsite:  (a)The Roofing and Construction, LLC at Richardson Hall - 414 East Street, Vermillion, SD 57069:  On or about and at times prior to July 10, 2020 the employer did not ensure that a fire extinguisher was provided within 50 feet of the jobsite located on the roof of the Richardson Hall dormitory while performing roofing work.  The crew was utilizing various flammable chemicals to include, but not limited to, Versigard G200SA Yellow substrate adhesive on the roof for adhering roofing materials.  This condition exposed employees to fire hazards.

29 CFR 1926.151(a)(3): Smoking was not prohibited at or in the vicinity of operations which constituted a fire hazard:  (a)The Roofing and Construction, LLC at Richardson Hall - 414 East Street, Vermillion, SD 57069:  On or about and at times prior to July 10, 2020 the employer did not ensure that smoking was prohibited on the roof where roofing work was being accomplished and utilizing various flammable liquids and adhesives.  This condition exposed employees to fire hazards.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  (a)The Roofing and Construction, LLC at Richardson Hall - 414 East Street, Vermillion, SD 57069:  On or about and at times prior to July 10, 2020 the employer did not ensure employees were protected from falls while working near the edge of a four story dormitory preforming roofing work.  This condition exposed employees to a fall hazard of 40 feet.
