29 CFR 1904.29(b)(1):  The Log of all Work-Related Injuries and Illnesses (OSHA Form 300) and/or the Summary of Work-Related Injuries and Illnesses (OSHA Form 300-A), and/or equivalent forms were not recorded in detail required by the respective forms:    a.)   Camboro Veterinary Hospital, on or about July 16th, 2020 and times prior thereto:  The OSHA Form 300 for calendar year 2020 did not provide specific detail about the object/substance that directly injured or made the employee ill, in Column F.  Case number 8, did not provide specific detail about the object/substance that directly injured or made the employee ill.

29 CFR 1910.1096(n)(1): The employer did not advise each employee of individual radiation exposure on at least an annual basis:     A.)  Camboro Veterinary Hospital, on or about July 16th, 2020 and times prior thereto:  Employees worked with and around radiation during procedures including, but not limited to, performing x-rays on various animals at the facility.  Employees were not advised of their radiation exposure records at least annually.    ABATEMENT CERTIFICATION REQUIRED:  Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
