On July 25, 2020 at 6110 Tricia Avenue in Weston, Wisconsin, employees were working on a roof surface nine feet above surface level without utilizing fall protection equipment.

29 CFR 1926.503(a)(2)(iii): The employer shall assure that each employee has been trained, as necessary, by a competent person qualified in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used. On July 25, 2020 at 6110 Tricia Avenue in Weston, Wisconsin, the employer did not provide required fall protection safety training for employees who did not associate fall from elevation hazards when working on a roof surface nine feet above surface level without utilizing fall protection equipment.
