29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) Front of Building - On or about July 15, 2020, employees were exposed to the hazard of falling while working without fall protection from a flat roof surface having a ground to eave height of (one story) approximately 12 feet. b) Sides of Building - On or about July 15, 2020, employees were exposed to the hazard of falling while working without fall protection from a pitched roof surface having a ground to eave height of (two stories) approximately 22 feet. No abatement certification or documentation required for this violation.

In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
