OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that an employee was exposed to struck by hazards and death from falling or swinging tree sections:  Worksite - On or about July 14, 2020, an employee was hospitalized after being struck by a limb that he was cutting.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in an in-patient hospitalization, amputation or the loss of an eye:    Worksite - On July 14, 2020, at approximately 11:00 am, an employee was struck by a tree branch and hospitalized. The Employer did not report the hospitalization until July 16, 2020, at 2:00 pm.
