29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift:  a)  922 Yellowstone River RD:  On or about July 17, 2020, and at times prior, employees were exposed to cave in hazards while performing oil line repairs in an unprotected trench, approximately seven foot deep. The employer failed to have a competent person inspect the trench prior to the start of work and as needed throughout the project to identify deficiencies.
