29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:    A) On or about July 22, 2020, at 84 Valley Road, Killingly, CT job site:  The employer did not utilize ground-fault circuit interrupters when temporary wiring was in use.  An electrical receptacle outlet on the construction site, which was not a part of the permanent wiring of the structure and was being used to power portable electric equipment, was not protected by a GFCI.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations:      A) On or about July 22, 2020 at 84 Valley Road, Killingly, CT job site:  The employer did not provide protection to employees who were working in a trench that was over 41 feet long and more than 5 feet deep by keeping all excavated soil and equipment at least 2 feet away from the edge of the trench.  Spoil piles were stored at or near the edge of the trench with no means in place to keep the loose material from falling into the trench.
