29 CFR 1910.67(c)(2)(ii): Extensible and articulating boom platforms; only trained persons shall operate an aerial lift.    On July 22, 2020 at 228 E Division Street in Fond du Lac, Wisconsin, the employee operating a vehicle mounted articulating boom platform was not trained to ensure the use of fall protection equipment during work from the platform at heights greater than twenty feet.

29 CFR 1910.67(c)(2)(v): Extensible and articulating boom platforms; a personal fall arrest or travel restraint system that meets the requirements in 29 CFR part 1910, subpart I, shall be worn and attached to the boom or basket when working from an aerial lift.  On July 22, 2020 at 228 E Division Street in Fond du Lac, Wisconsin, an employee was working from a vehicle mounted Altec Articulating boom platform twenty feet above surface level without utilizing fall protection equipment.
