As a result the employee suffered the partial amputation of three fingers that were caught and run through 2 metal gears.

29 CFR 1910.147(c)(5)(ii)(C)(1): Lockout devices were not substantial enough to prevent removal without the use of excessive force or unusual techniques, such as with the use of bolt cutters or other metal cutting tools:  (a)  Facility:  On or about July 27, 2020, all of the facilities locks that could be used for locking out of equipment were keyed the same.  All of the keys and locks were kept in an unlocked steel cabinet at the back of the facility.  Once an authorized employee placed a lock on a machine to lock it out anyone could have retrieved a key from the steel cabinet and taken the authorized persons lock off of the machine.
