29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    On or about July 28, 2020, employees were not protected from falling through unguarded skylights, exposing employees to a fall of greater than 9 feet.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d)

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.    On or about July 28, 2020, the employer did not certify fall training for employees performing roofing operations at heights exceeding six feet.      Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
