On or about 07/29/2020, at 3800 Deer Drive, Sarasota, FL 34240, employees were exposed to a fall hazard while working from a tubular welded frame scaffold 4 frames high and 7 frames wide, in that, 5 scaffold platforms were not fully planked as required allowing a 12 inch distance to the scaffold upright.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a. On or about 07/29/2020, at 3800 Deer Drive, Sarasota, FL 34240, employees were exposed to a fall hazard while working from a tubular welded frame scaffold 4 frames high and 7 frames wide, in that, the scaffold was not provided with guardrails as required. The scaffold top frames (fourth frame) were missing the guardrails at the ends and at 6 other frames, the third frames were missing the guardrails at the ends and at 3 other frames and the second frames were missing the guardrails at the ends and at 3 other frames.
