29 CFR 1910.332(b)(2): Employees, who were not qualified persons, were not trained in and familiar with any electrically related safety practices not specifically addressed by 29 CFR 1910.331 through 29 CFR 1910.335, but which were necessary for their safety: (a) Facility, 228 Aladdin Road, Leechburg, PA: On or about July 29, 2020 training had not been provided to employees considered to be "unqualified" to ensure they were protected from electrical hazards. An employee was positioned in front of an open electrical panel when another person pressed a control button and an arc occurred hitting the employee with hot metal and causing temporary hearing loss and scratched corneas and retinas. The employee had not received information and training on the hazards associated with electrical flash and protective equipment necessary when exposed to potential electrical hazards.

29 CFR 1910.333(a)(2): Where exposed live parts were not deenergized (i.e., for reasons of increased or additional hazards or infeasibility), other safety-related work practices were not used to protect employees who could be exposed to the electrical hazards involved: (a) Facility, 228 Aladdin Road, Leechburg, PA: On or about July 29, 2020 an electrical panel was not de-energized when it was opened revealing the live parts. An employee who was positioned approximately 20 inches from the panel was not protected from flash when another person pressed a button on the open door that activated a controller located inside. A failure occurred which caused a flash and the employee sustained burns, eye and ear injuries.
