Cherry Hill, NJ - The employer provided and required the use of respirators at the facility.  The employer did not ensure that employees, such as, but not limited to, housekeeping employees, Certified Nursing Assistants, Licensed Practical Nurses and Registered Nurses, who were provided with and required to wear N95 respirators, were provided with a medical evaluation prior to their use of respirators, on and after 5/2/2020.        ABATEMENT DOCUMENTATION AND CERTIFICATION IS REQUIRED FOR THIS ITEM
