29 CFR 1928.57(a)(6): At the time of initial assignment and at least annually thereafter, the employer did not instruct every employee in the safe operation and servicing of all covered equipment with which he was or would be involved: Location: Lower hay barn at 452 South Road, Somers, CT: Operators and those working in the vicinity of the GEHL AL 650 articulated loader and Mega Claw Model #BDGH42 attachment, had not been instructed in the safe operation of the machine , as detailed in the operators manual, to ensure they were knowledgeable in the hazards associated with its operation and the proper methods and procedures for operating in and around the machine safely.

29 CFR 1928.57(a)(6)(iv): Employer did not ensure everyone was clear of machinery before starting the engine, engaging power, or operating the machine: Location: Lower hay barn at 452 South Road, Somers, CT: On and before August 3, 2020, the established procedures for loading hay and straw into trailers with the GEHL AL 650 articulated loader did not ensure that everyone was clear of the machine throughout the operation. During the loading process, the operator was required to operate the piece of equipment with their front view completely obstructed by the hay and straw that was being moved, preventing them from ensuring that the path of travel was clear of anyone who might be in the area.
