29 CFR 1910.28(b)(7):  The employer did not ensure that each employee on a walking-working surface near an opening, including one with a chute attached, where the inside bottom edge of the opening is less than 39 inches (99 cm) above that walking-working surface and the outside bottom edge of the opening is 4 feet (1.2 m) or more above a lower level is protected from falling by the use of: (i) Guardrail systems; (ii) Safety net systems; (iii) Travel restraint systems; or, (iv) Personal fall arrest systems.  a) Chute - Employees clearing debris in a garbage disposal chute were exposed to a fall hazard of approximately ten (10) feet without the use of a conventional fall protection system, on or about July 30, 2020.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):  a) Compacter 1 - Employees clearing a jam in a chute on the conveyor were exposed to caught-in hazards from not isolating all stored energy from the trash compactor running below the chute, on or about July 30, 2020.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation:  a) Total Recycle - The employer did not list object or substance that directly injured or made the person ill, in column F for each employee listed on the 300 log for 2020.  The last injury documented before inspection was on July 27th, 2020.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
