29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   On or about 7/28/2020 at 3100 Zingara Road Conyers, GA where the employer failed to protect workers, installing CMU block and mortar from the top platform of the scaffold during access and egress, from falls above 11-feet to the lower level where the level was not protected with guardrails or Crossbraces.

29 CFR 1926.452(c)(4): End frames of tubular welded frame scaffolding were not locked together vertically by use of pins or equivalent means in areas where uplift could occur:  On or about 7/28/2020 at 3100 Zingara Road Conyers, GA where the employer failed to ensure that the scaffold was properly erected with connecting pins installed where a rough terrane forklift was observed delivering a trough of mud onto the scaffold.

On or about 7/28/2020 at 3100 Zingara Road Conyers, GA, where the employer failed to ensure workers at 11-feet above the working surface were protected against falling while laying mud for the block laying activities.
