29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person:  Star Metals Apartment/Office Building located at 1041 Howell Mill Road NW, Atlanta, GA 30312, 8th floor: On August 4, 2020, the employer did not ensure the mobile fabricated frame scaffold was erected, moved, dismantled, or altered under the direct supervision and direction of a competent person qualified in scaffold erection. Employees worked from mobile fabricated scaffold platforms that extended past the base of the caster wheel supports creating a tip-over/fall hazard to the lower level.

29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:  Star Metal Apartments located at 1041 Howell Mill Road NW, Atlanta, GA 30312, 8th floor: On August 4, 2020, the employer exposed employees to tip-over/fall hazards of approximately 18-feet to the lower level, in that the caster wheels supporting the fabricated frame scaffold used to perform stucco work on the soffit were not locked in place prior to the start of work.

29 CFR 1926.452(w)(7): The mobile scaffold platform extended outward beyond the base supports of the scaffold and outrigger frames or equivalent devices were not used to ensure stability:  Star Metal Apartments located at 1041 Howell Mill Road NW, Atlanta, GA 30312, 8th floor: On August 4, 2020, the employer exposed employees to tip-over/fall hazards of approximately 18-feet to the lower level, in that scaffold extensions were used on a caster wheel supported scaffold which was not provided with devices such as counterweights or outrigging systems to prevent tip over or collapse of the scaffold system used to perform EIFS installations on overhead soffits.

The fabricated frame mobile scaffold being used by employees had scaffold platform extensions which cannot be worked on when they pass the base.
