29 CFR 1926.502(d)(11): Lifelines were not protected against being cut or abraded: a) On or about August 5, 2020, and at times prior, Total Welding, Inc. did not ensure that lifelines were protected against being cut or abraded. A vertical lifeline used by Total Welding, Inc. employees was set up using a 3/8" galvanized steel cable wrapped around 90 degree corners over the top of a concrete masonry unit wall without any protection against cuts or abrasion exposing workers to fall hazards greater than 15 feet. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways: a) On or about August 5, 2020, Total Welding, Inc. did not ensure that each employee using ladders had been trained by a competent person in the proper use, placement, and care in handling the ladders. A Total Welding, Inc. employee was exposed to fall hazards greater than 10 feet. The worker was setting up an extension ladder at an unprotected edge on the top tread of the south core stairway. The worker lost his balance and fell to a lower level sustaining fatal injuries. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
