29 CFR 1926.1060(a)(1)(iii): The employer did not ensure that each employee using ladders and/or stairways had been trained by a competent person in the proper construction, use, placement, and care in handling the ladders and/or stairways:   a) On or about August 5, 2020, Total Welding, Inc. did not ensure that each employee using ladders had been trained by a competent person in the proper use, placement, and care in handling the ladders. A Total Welding, Inc. employee was exposed to fall hazards greater than 10 feet. The worker was setting up an extension ladder at an unprotected edge on the top tread of the south core stairway. The worker lost his balance and fell to a lower level sustaining fatal injuries.
