29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    Production Area, DoAll Metal Horizontal Band Saw, Machine #191.  On or about August 5, 2020, an employee did not shut down, deenergize and lock out the saw prior to beginning a servicing and maintenance task involving removal of a guard and inspection of the power transmission belt and pulley, in an attempt to determine the cause of the malfunctioning of the saw.  The employee was exposed to amputation and laceration hazards from the moving belt and ingoing nip point between the belt and pulley.   Hazardous energy sources were not controlled in the following sequence:          1) turn off and shut down the machines,       2) physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s),  3) affix lockout or tagout devices to each energy isolating device,       4) relieve, disconnect, restrain and otherwise render safe all hazardous residual or stored energy,     5) verify that isolation and deenergization of the machines or equipment have been accomplished.        Alternative protections, such as machine guarding, were not utilized.   Abatement documentation is required for this item in accordance with the requirements of 1903.19(d).
