29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  A) Blair Masonry on or about August 12, 2020 at the address of 114 South Center Street, Grove City, PA:  On or about 8/12/2020, Employees were conducting masonry operations from a tubular welded frame scaffold.  A single 2 X 8 board was placed on the scaffold that employees were walking on to access from one scaffold platform to another.  The space between the 2 X 8 board and the scaffold unit was at least 12-inches on both sides.  The condition exposed employees to a fall hazard of 12-feet.
