A) Blair Masonry on or about August 12, 2020 at the address of 114 South Center Street, Grove City, PA: On or about 8/12/2020, Employees were conducting masonry operations from a tubular welded frame scaffold. A single 2 X 8 board was placed on the scaffold that employees were walking on to access from one scaffold platform to another. The space between the 2 X 8 board and the scaffold unit was at least 12-inches on both sides. The condition exposed employees to a fall hazard of 12-feet.
