29 CFR 1910.269(d)(6)(vii): Before starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergizing of the machine or equipment had been accomplished.  On or about August 17, 2020,  near the back yard easement of private residential homes located in Kingwood, TX, employees were exposed to electrical hazards when working on or near an energized 19.9 kilovolt power line, without verifying that it was de-energized.
