Employees were not wearing eye and face protection. a) Building Exterior-left side of the building: Employees were exposed to eye/face injuries while mixing/applying mortar to brick joints without the use of eye and face protection. 29 CFR 1926.451(e)(1): scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, employees were not using portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Cross braces or scaffolds shall not be used as a means of access. a) Building Exterior-right side: Employees were using a scaffold that the platform was more than two feet above the access point. Employees were not using portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure or personnel hoist, on or about 8/17/20.
