29 CFR  1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees.   WORKSITE: Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to the employee using a wall-mounted breaker box with an unsecured nonmetallic-sheathed cable causing the cable to rub against the metal corner of the box creating a fire hazard. National Electric Code (2017) 334.39 Securing and Supporting requires nonmetallic-sheathed cables entering enclosures to be secured within 12 inches of entry which would prevent the hazard.

29 CFR  1910.303(g)(1)(i)(A): The depth of the working space in the direction of access to live parts may not be less than indicated in Table S-1. Distances shall be measured from the live parts if they are exposed or from the enclosure front or opening if they are enclosed.  WORKSITE: The employer did not maintain a minimum working clearance of three (3) feet in front of the wall mounted breaker box. The front of the box was obstructed by a microwave, mini-refrigerator, and plastic cups.  The area in the direction of access to the live parts enclosure contained a five-gallon bucket with mop and broom heads, a plastic crate, a mini-fridge, and a small microwave. These materials were stored directly in the direction of access to the breaker box and there was no clear working space. Materials adjacent to the breaker box may also become energized during access to the breaker box, including a metal pole which protrudes from the 5-gallon bucket to within approximately one foot of the breaker box.
