29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about August 18, 2020, an apartment reroofing project located at 2800 N.W. 192nd Street in Oklahoma City, OK, employees were allowed to use stairway openings that did not have guardrails around all open ends, exposing employees to the hazard of falls.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: On or about August 18, 2020, at an apartment reroofing project located at 2800 N.W. 192nd Street in Oklahoma City, OK, employees were allowed to use ladders that did not extend at least 3 feet beyond upper the landing of stairways in an apartment building, exposing the employee to the hazard of falls.
