29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   At the jobsite located at 4400 NE 88th St., Kansas City, Missouri- Employees were observed using pneumatic nail guns and were not protecting their eyes by the use of any means of eye protection.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  At the jobsite located at 4400 Northeast 88th Street, Kansas City, Missouri- Employees were exposed to electrical hazards in that the yellow extension cord plugged into the generator, and an orange and black extension cord had their outer insulation damaged.

29 CFR  1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  At the jobsite located at 4400 Northeast 88th Street, Kansas City, Missouri- Employees were exposed to electrical hazards in that a red and black extension cord plugged into another extension cord had a broken strain relief.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  Worksite located at 4400 NE 88th Street, Kanas City, Missouri:  Employees were exposed to fall hazards.  Employees engaged in construction activities were exposed to serious injuries, in that a Caterpillar TL642C forklift was being used to support an elevated makeshift work platform.  The platform was a job made box that was not specifically designed and approved by the manufacturer for this task.
