29 CFR 1910.138(b): Selection. Employers shall base the selection of the appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the task(s) to be performed, conditions present, duration of use, and the hazards and potential hazards identified. On or about August 14, 2020, at the Central Staging Platform, the employer did not ensure that employees used the appropriate hand protection relative to the task(s) being performed. This condition exposed employees to struck-by and caught-between hazards.

29 CFR 1910.145(d)(4): Caution signs. Standard color of the background shall be yellow; and the panel, black with yellow letters. Any letters used against the yellow background shall be black. The colors shall be those of opaque glossy samples as specified in Table 1 of American National Standard Z53.1-1967. On or about August 14, 2020, at the Altet platform loader, caution signs for potential pinch point hazards were not posted. This condition exposed employees to struck-by and caught-between hazards.
