29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   On or about August 21, 2020, and at times prior thereto, employees worked installing sheathing on a residential home from an aerial lift without the use of a body belt and lanyard, exposing employees to fall and ejection hazards of approximately 15-feet to the ground below.
