29 CFR  1910.178(l)(4)(ii)(B):Refresher training in relevant topics was not provided to the operator when the operator was involved in an accident or near-miss incident:  a) 141 Wideawake Street, Pine Grove, PA 17963: An interior wall in the back of the warping department near the bathrooms had been damaged by an employee operating a forklift. Refresher training in relevant topics in Powered Industrial Trucks was not provided to the operator that was involved in the accident on or about August 25, 2020.
