Veterans Memorial Parkway, Warrenton, MO, employees were applying shingles using a Hitachi Nail Gun, Serial #NV45AB2, 1 3/4 Coil Nailer, Air Pressure 70 - 120 psi, and were not wearing safety glasses.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about 08/24/2020, at the jobsite at 1204 E. Veterans Memorial Parkway, Warrenton, MO, employees engaged in roofing installation operations for a commercial loan building and were not utilizing conventional fall protection. The ground to eave height was approximately 11' 9".

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder: On or about 08/24/2020, at the jobsite at 1204 E. Veterans Memorial Parkway, Warrenton, MO, workers were using an aluminum ladder to gain access to the roof, the ladder's length did not allow the ladder side rails to extend at least 3 feet above upper landing surface, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device, such as a grab rail, was not provided to assist employees in mounting and dismounting the ladder exposing them to a potential fall hazard. The ground to eave was 11' 9".
