29 CFR 1910.28(b)(3)(i): The employer did not ensure each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following: covers, guardrail systems, travel restraint systems, or personal fall arrest systems, as required by sub-items A through D of this section.    A) Poet Biorefinery-Marion LLC, 1660 Hillman-Ford Road, Marion, OH, 43302:  On or about August 21, 2020, the employer did not ensure employees were protected from falls while performing bearing inspections on Scalper 2 level.  Employee was performing these activities near an unguarded open floor hole and was exposed to a fall of 8 feet to a metal grating surface on level below.

29 CFR 1904.29(b)(1): The employer did not provide the required injury or illness descriptions on the Log of Work-Related Injuries and Illnesses, OSHA Form 300:    On or about August 27, 2020, the employer failed to ensure that the OSHA Form 300 for calendar year 2020 contained a complete description in column F.  The 2020 300 log did not define the specific injury sustained by the employee
