29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a. On or about 08/26/2019, at the job site - employees were exposed to a head injuries, in that, hard hats were not being used while conducting roofing operations.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   a. On or about 08/26/2019, at the job site - employees were exposed to a fall as high as 15 feet while conducting roofing work, in that, fall protection was not used.  The JWA Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1354794, citation number 1, item number 1and was affirmed as a final order on 2/25/2019, with respect to a workplace located at 487 W Artesia St, Orlando, FL 32765.  The JWA Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1358884, citation number 1, item number 1and was affirmed as a final order on 5/17/2019, with respect to a workplace located at_2304 Curzon Way, Odessa, FL 33556.  The JWA Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1397162, citation number 1, item number 1 and was affirmed as a final order on 9/28/2019, with respect to a workplace located at 807 Silver Rose Ct, Lake Mary, FL 32746.  The JWA Contracting, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1366403, citation number 1, item number1and was affirmed as a final order on 6/4/2019, with respect to a workplace located at 13121 Kingshyre Way, Tampa, FL 33610.
