OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees that were exposed to struck by hazards from an unguarded infeed chain conveyor. On or about August 31, 2020 employees were exposed to struck by hazards from lumber falling off the infeed chain conveyer at the point where bundles of lumber are loaded onto it. Recommended Abatement Action Among other methods, one feasible and acceptable method to correct this hazard is to follow the recommendations of ANSI/ASME B20.1-2012, Safety Standard for Conveyors and Related Equipment, including but not limited to the following section: 5.13 Transfer, Loading, and Discharge Points (a) At transfer, loading, and discharge points where unconfined and uncontrolled free fall of material can result from flooding, ricocheting, overloading, trajectory, leakage, or a combination thereof, such unconfined and uncontrolled free fall of material shall be prevented if it would create a hazard to personnel. (b) In the absence of a guard or barrier specifically erected to protect personnel, warnings shall be provided to restrict unauthorized personnel from entering hazardous loading, unloading, and transfer areas.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees that were exposed to pinch point hazards. On or about August 31, 2020 at the front of the infeed chain conveyor, employees are exposed to in-going nip points when the infeed chain conveyer is remotely started as they stand close to the infeed conveyer to cut banding on bundles of lumber. Recommended Abatement Action Among other methods, one feasible and acceptable method to correct this hazard is to follow the recommendations of ANSI/ASME B20.1-2012, Safety Standard for Conveyors and Related Equipment, including but not limited to the following section: 1. Ensure employees follow the practice(s) specified or alternatively ensure the conveyer is equipped with a pre start signal as set forth in section 5.11.2(b) :A conveyor that would cause injury when started shall not be started until personnel in the area are alerted by a signal or designated person that the conveyor is about to start. 2. Ensure employees follow the practice(s) specified or alternatively ensure the conveyor is equipped with an emergency stop device as set forth in section 5.11.2(c): Remotely and automatically controlled conveyors and conveyors where operator stations are not manned or are beyond voice or visual contact from drive areas, loading areas, transfer points, and other potentially hazardous locations on the conveyor path not guarded by location, position, or guards shall be furnished with emergency stop buttons, pull cords, limit switches, or other emergency stop devices.
