29 CFR 1926.28(a): Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions:  (a) 69 Page Lane  Phoenixville, PA: On or about August 28, 2020, an employee was not wearing head protection while roofers worked above, replacing roof shingles on a two-story residential structure.  Tonys Master Construction LLC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.28(a) WHICH WAS CONATAINED IN OSHA INSPECTION NUMBER 1427010, CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON 29 Aug 2019, WITH RESPECT TO A WORKPLACE LOCATED AT 2285 Westminster Dr. Emmaus, PA. 18049.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) 69 Page Lane  Phoenixville, PA, Worksite - On or about August 28, 2020, employees were performing residential roofing work, approximately 20 feet above grade, without utilizing any means of fall protection.     Tonys Master Construction Llc., Was Previously Cited For A Violation Of This Occupational Safety And Health Standard Or Its Equivalent Standard 29 Cfr 1926.501(B)(13) Which Was Conatained In Osha Inspection Number 1427010, Citation Number 1, Item Number 2, And Was Affirmed As A Final Order On 29 Aug 2019, With Respect To A Workplace Located At 2285 Westminster Dr.  Emmaus, Pa. 18049. 29 CFR  1926.1053(b)(10):The tops of a non-self-supporting ladder with more than one support attachment were not placed with the two rails supported equally:  (a) 69 Page Lane  Phoenixville, PA.: On or about August 28, 2020, employees used two 24-foot Werner extension ladders to access the roof of a two-story residential structure, but both ladders had side rails that did not extend at least 3 feet above the upper landing surface, exposing employees to ladder misuse and hazards.  Tonys Master Construction LLC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.1053(b)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1427010, CITATION NUMBER 1, ITEM NUMBER 3, AND WAS AFFIRMED AS A FINAL ORDER ON 29 Aug 2019, WITH RESPECT TO A WORKPLACE LOCATED AT 2285 Westminster Dr. Emmaus, PA. 18049.
