29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder.  a.  On or about September 1, 2020, and at times prior, at 377 Stonehurst Parkway, at the roof access ladder, three roofing employees were exposed to fall and ladder collapse hazards in that the roof access ladder was placed at 60 degrees,  a shallow angle of repose.  b.  On or about September 1, 2020, and at times prior, at 383 Stonehurst Parkway, at the roof access ladder, two roofing employees were exposed to fall and ladder collapse hazards in that the roof access ladder was placed at 55 degrees, a shallow angle of repose.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a.   On or about September 1, 2020, at 377 Stonehurst Parkway, on the 6:12 pitch roof slopes, three roofing employees installing roofing paper were exposed to an 8 feet fall hazard from the eaves in that they were not protected by a fall protection system.  b.   On or about September 1, 2020, at 383 Stonehurst Parkway, on the 6:12 pitch roof slopes,  two roofing employees installing roofing shingles and drip edging metal were exposed to 8 feet and 14 feet fall hazards from the eaves in that they were not protected by a fall protection system.  Alton Roofing Services, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1448962, citation number 1, item number 1 and was affirmed as a final order on March 27, 2020, with respect to a workplace located at 9525 Staple Mill Drive, Jacksonville, Florida.
